  2. DESKRIPSI • Rentang Kategori Neoplasma tersebar dari C00-D48. • 136 dari 149 kategori telah dialokasikan. • Ada 7 blok. Dengan blok kode malignansi primer yang mempunyai subdivisi 12 sub-blok. • Sangat penting bahwa pada pengkodean Neoplasm harus menggunakan buku Volume 3 dan 1 agar bisa menemukan kode yang tepat. • Kode M..../. Harus di cari di Bagian Morphologi of Neoplasms.

  3. TUJUAN Pengkode mampu mengenal: - behaviour (sifat tumor) neoplasmanya: malignant, benign, in situ dan uncertain - anatomical site (site anatomik) - primary site or metastasis (site primer atau metastasis) - kode morfologi sel tumornya (M.../.) - cara mencari kode melalui Volume 3 dan Volume 1, dan menemukan kode M..../. -nya. - perbedaan ICD-10 Bab II dari ICD-O

  4. Contoh: Malignant adeno-carcinoma of breast (primary site) with secondaries to lung. (adenocarcinoma kelenjar mamma menyebar ke paru). Pada payu (breast) (primer): C50.9 M8140/3 Pada paru (lung) (sekunder): C78.0 M8140/6

  5. Site of Neoplasm • Titik mula tumor tumbuh = Primary site • Harus dikode akurat dan serinci mungkin • Organ penerima anak sebar tumor terkait disebut tumor metastases atau tumor sekunder. • Kode tumor metastasis kurang spesifik • Apabila site diagnosis tidak dispesifikasi apakah site primer atau sekunder, kita anggap ia primer.

  6. Morphology • Cari kode M..../. pada table di Bab Morphology of Neoplasms ICD-10 Volume 1. [Hal.1179 – 1204] • M..../ adalah klasifikasi tambahan untuk merinci bentuk dan struktur neoplasmnya. • Kode M..../ tidak mandatoris (diwajibkan) • Penerapan kode M harus didukung ada bukti hasil pemeriksaan sitologi PA.

  8. Tabel Neoplasms • Cari di Volume 3 melalui kata panduan alfabet ‘N’  Neoplasms Daftar tersusun secara alfabetik dan tertata dalam 5 kolom/lajur • Masing lajur tersedia sesuai sifat (behaviour) tumornya dan sesuai kode yang tertera di Bab II Neoplasm.

  9. Pemanfaatan Indeks • Tipe morfologi seluruhnya di-indeks di Volume 3 Kode sifat (behaviour) ada di bagian akhir kode M, sehingga membantu penentuan kode mana dalam tabel neoplasms yang akan digunakan. • Pada keadaan tertentu, kedua Kode M dan kode Site tumornya disertakan dalam Index apabila tumor terkait hanya muncul pada satu site. • Penelusuran organ yang terkena neoplasm harus lewat Neoplasm di Volume 3.

  10. Lead-terms • Tumor (548; 596) Tumor (M8000/1) – see laso Neoplasm uncertain behaviour Di bawah Tumor tidak dapat ditelusuri site lokasi tubuh yang terkena tumor yang dimaksud. M8000/1 adalah kode morfologi untuk tumor yang sifatnya belum dapat dipastikan. Perhatikan keterangan di Bab Morphologi Neoplasms di Vol.1 [1179-1204; 1137-1159]

  11. Neoplasm (369-401; 402-437) • Apabila letak tumor diketahui  dari istilah organ tubuh yang terkena tumor , sesuai perintah see also Neoplasm di ICD-Vol. 3. Tabel di bawah Neoplasm disusun alfabetis sesuai lokasi/site tubuh yang terkena neoplasm. • Tabel terinci dalam 5 lajur, untuk Malignantprimary atau sekunder, in situ,benign atau uncertain/unknown behaviour.

  12. Cara memilih lajurnya • Perhatikan kode M..../. Tumornya.[1179; 1138] /0 Benign D10-D36 /1 Neopl. of uncertain and .... D37-D48 /2 Insitu D00-D09 /3 Malignant, primary C00-C76 C80-C97 /6 Malignant , secondary C77-C79 /9 Malignant uncertain primary or secondary Ada Except di baris bawah yang harus diperhatikan!

  13. Pada ICD-ONCOLOGY (ICD-O) /9 Malignant uncertain primary or secondary tidak aplicable dalam kontekICD-0 (ed. 1900) Dalam ICD-O semua neoplasm malignant dipastikan (presumed) adalah - primer (/3) atau - sekunder (/6), sesuai informasi lain-lain yang terdokumentasi di Rekam Medis pasiennya.

  14. Pada ICD-O (lanjutan) Pada indeks alfabetis ICD Vol.3, memuat semua sinonim yang ada di ICD-O berikut nomor kode deskripsi morfologi lain yang mungkin ada di rekam medis pasien, namun dihilangkan dalam ICD-O sebagai yang: - outdated (kuno) atau - undesirebale (tidak diinginkan lagi).

  15. Neoplasm yang site spesifik • Beberapa tumor site yang spesifik: Contoh di antaranya: - nephroblastoma C64 [208; 192] - hepatocellular carcinoma C22.0 [193; 177] - basal cell carcinoma C44.- [200; 184] Bagi istilah tersebut, kode pada Bab II diberi tambahan tanda dalam kurung ( ) untuk nomenklaturnya (lihat di masing hamalan Vol.1)

  16. Kanker (Cancer), Karsinoma (Carcinoma) dan Sarkoma (Sarcoma) • Ketiga sebutan di atas adalah tumor ganas (malignant) • Di bawah masing sebutan: (ICD-Vol.3) Cancer (84; 91) Perhatikan Note yang ada. Carsinoma (85; 92) Perhatikan Note yang ada. dan Sarcoma (485; 527),  tidak dapat ditemukan site organ yang terkena melalui urut alfabet, daftar hanya memuat tipe selnya (morfologinya), kadang disertai site umumnya  mencari site khusus  see also Neoplasm.

Baca aturan pengkodean morfologi di [1179-1180; 1138] tentang digit ke-5 pada Morfologi tumor Remember that it is possible to change the behaviour (that is the 5th digit of the M code) to suit the diagnosis you have been given.
