  2. ผู้ป่วยเด็กชายไทย อายุ 40 วัน ภูมิลำเนากรุงเทพมหานคร • อาการสำคัญไข้ ไอ หอบ มา 1 สัปดาห์ • ประวัติปัจจุบัน • 3 สัปดาห์ก่อนมาโรงพยาบาล (ขณะอายุ 20 วัน) เริ่มมีไข้ตอนกลางคืนไอแห้ง ๆ ไม่มีน้ำมูก ไม่ซึม ยังดูดนมได้ดี • 1 สัปดาห์ มีไข้ต่ำ ๆ มาตลอด ไอมีเสมหะ มารดาพาไปโรงพยาบาล แพทย์ให้ยาลดไข้และยาแก้ไอ ตอนเย็นเด็กเริ่มหายใจเร็ว จึงพาไปคลินิก ได้ยาลดไข้ แก้ไอ และยาแก้อักเสบ

  3. ประวัติปัจจุบัน (ต่อ) • 5 วันก่อนมาโรงพยาบาล ผู้ป่วยยังคงมีไข้ตลอด ไข้สูงเกือบทั้งวัน ไอมีเสมหะ หายใจหอบ ซึมลง ไม่ยอมดูดนม จึงพาไปโรงพยาบาลอีกครั้ง

  4. CBC: Hb= 10, Hct= 31%,WBC= 51,730/mm3(N51,L34), Plt= 944,000 , • CXR พบ consolidation with right pleural effusion • pleural tapping ได้ frank pus รักษาโดยCefotaxime (100MKD) ร่วมกับ Amikin (15MKD) ทางหลอดเลือดดำ และใส่สายระบายหนอง (ICD) ผู้ป่วยยังคงมีไข้ หายใจหอบเหนื่อยมากขึ้น ต้องใส่ endotracheal tube และเครื่องช่วยหายใจ จึงส่งมารักษาต่อที่วชิรพยาบาล

  5. ประวัติอดีต • เป็นบุตรคนแรก คลอดครบกำหนด APGAR 9,10 น้ำหนักแรกคลอด 3,000 กรัม หลังคลอดอยู่โรงพยาบาล 2 วัน • กินนมแม่อย่างเดียว • ได้รับวัคซีนครบตามกำหนด • มีคนอยู่ที่บ้าน 10 กว่าคน ทุกคนแข็งแรงดี

  6. Physical examination • T 37.4oC , HR 140 /min , RR 56 / min , BP 60 / 37 mmHg ,AF 1.5*1.5 cm not bulging , BW=3.6 Kgs • GA : looked sick , dyspnea • HEENT : not pale conjunctiva , no icteric sclera , no injected pharynx and tonsils, lymph nodes are not enlarged • Heart : regular , no murmur, good pulses, capillary refill 1 sec

  7. Lungs : decreased breath sound ,right dullness on percussion ,on ICD at right 5th intercostal space • Abdomen : soft , liver and spleen can’t be palpable • Extremities : no pitting edema

  8. CBC : Hb 9.1 gm / dl , Hct 36% , WC 46,100 / mm3 ( 84% , L 9% , B 2% , M 5%) ,Platelet 242,000 / mm3 • Urine examination : yellow, clear, pH 1.009 , wbc 4-5 / HPF , rbc 0-1 / HPF, albumin 1+

  9. Blood chemistry : BS 136 , BUN 8 , Cr 0.4 , Na 135 , K 3.9 , CI 101 , HCO3 19 • Sputum and pleural fluidgram stain: gram positive cocci in chain

  11. LP: WBC 2/mm3, Protein 60 , sugar 49 (108) • CSF C/S : NG • H/C : NG

  12. PPD test : 10 mm • Gastric lavage for AFB 3 days : neg • Sputum for AFB : neg for TB
